Job summary

Curana Health, Inc. is seeking a Care Ally, Case Manager to join our interdisciplinary team. You will assess member needs, develop and monitor care plans, educate members and families, and coordinate with care teams, PCPs, and community resources to deliver patient-centered care.

We value autonomous, hybrid work arrangements, travel to facilities 2–3 days weekly, and experience in case management or home health. RN/LPN or social work backgrounds are encouraged to apply.

Qualifications

• One year of clinical practice in case management, home health, or related fields.
• Proficiency with Microsoft Office and navigating multiple systems.
• Case management certification preferred.
• Long-term care or nursing facility experience preferred.

Responsibilities

• Executes strategies to improve member experience and care outcomes.
• Contacts plan members to assess health and develop a care plan.
• Educates members and families about diseases, plan benefits, and resources.
• Collaborates with interdisciplinary care team and PCPs for appropriate treatment.
• Follows up with members to monitor progress and adjust plans.
• Assists with scheduling, appointments, and care coordination.
• Coordinates home health visits and durable medical equipment requests.
• Documents care team meetings and transmits updates to plans.
