Job summary

P3 Health Partners is seeking an RN Care Manager to oversee comprehensive care for defined patient populations. The role blends in-office and remote work, three days per week in the office, with focus on planning, coordinating and evaluating care across the continuum.

The ideal candidate will have strong clinical judgment, care-management experience and excellent communication. RN licensure in applicable states and a nursing degree are required, with a salary range of $85,000–$95,000 depending

Qualifications

• RN licensure in NV, AZ, OR or CA with ability to obtain in two years.
• Bachelor's degree in Nursing preferred.
• Experience in case management or utilization review preferred.
• Strong communication and organizational skills.

Responsibilities

• Promote the mission and values of P3 Health Partners.
• Perform comprehensive patient assessments through records and interviews.
• Develop, implement and evaluate patient care plans.
• Coordinate care for high risk patients with care teams.
• Document timely per policy and manage gaps in care.
