Job summary

RCM Health Care Services is seeking an RN Case Manager – Behavioral Health in Brooklyn, NY. The role combines telephonic and in-person care coordination to support members across the continuum of care.

You will assess, plan, advocate, and authorize services while collaborating with families, providers, and interdisciplinary teams to promote independence and positive outcomes. Hybrid/field-based work is available.

Qualifications

• Requires an active NY RN license or eligible for licensure in NY.
• Associate Degree in Nursing is required; a BSN is preferred.
• Ability to coordinate care across multiple settings and teams.

Responsibilities

• Conduct assessments, care planning, and ongoing care management across hospital, sub-acute, long-term care, and home settings
• Coordinate services with members, caregivers, physicians, and interdisciplinary teams
• Perform home visits to assess living conditions and needs
• Develop and maintain Patient-Centered Service Plans and monitor eligibility
• Support discharge planning, transitions, and documentation per regulations
