P3 Health Partners is seeking an RN Care Manager to ensure comprehensive and coordinated care for assigned patient populations. The role involves nursing assessment, planning, implementation, and collaboration with medical management to reduce costs and improve outcomes.

The position supports a hybrid work arrangement with in-office days three days per week. Requires RN licensure in applicable states, proficiency with care management tools, and strong communication skills.

Qualifications

• Graduate from an Accredited School of Nursing or other applicable clinical role.
• Bachelor's degree in Nursing preferred.
• Certification in case management or ability to obtain within two years of employment.

Responsibilities

• Promote the mission, vision and values of P3 Health Partners.
• Coordinate comprehensive patient assessments through records, interviews, and telephonic or face-to-face encounters.
• Develop, implement and evaluate patient care plans.
• Collaborate with Medical Management on care plans and progress.
• Identify care needs and barriers; develop improvement plans.
• Prepare documentation per timelines and policy.
• Coordinate care for high-risk patient populations.
• Support self-management with patients and families to meet care goals.
• Build relationships with PCPs and other stakeholders.
• Address gaps in care.
