PrideGlobal is seeking an experienced Registered Nurse Care Manager for a hybrid role supporting older adults with complex healthcare needs in the Bronx, NY. The position blends field visits with remote care coordination and offers a 3-month on-site training period.

The ideal candidate holds an active NY RN license, an associates degree (BSN preferred), and at least 3 years of RN experience in MLTC, home care, geriatrics, or case management. Salary ranges from $100,000 to $130,000 per year.

Qualifications

• Active, unrestricted New York RN license.
• Associate’s degree in nursing; BSN preferred.
• Minimum 3 years RN experience in MLTC, home care, geriatrics, or case management.

Responsabilités

• Conduct in-home assessments to evaluate clinical, functional, and social needs.
• Develop and manage individualized care plans with physicians and interdisciplinary teams.
• Coordinate care across home care, hospitals, rehabilitation facilities, and long-term care settings.
• Complete approximately 6 home visits per week in the Bronx.
• Monitor member progress and adjust care plans to achieve optimal outcomes.
• Support discharge planning and help reduce hospital readmissions.
• Communicate with members, families, caregivers, and healthcare providers.
• Maintain accurate clinical documentation according to guidelines.
• Advocate for members and connect them with healthcare and community resources.
