Compass Healthcare Consulting & Placement seeks a Hybrid Remote & Field RN Care Manager for a Managed Care Plan serving New York. The role blends telephonic case management with one field visit per week in designated boroughs.

Candidates must hold a NYS RN license, have 2+ years of RN Case Management in MLTC or related settings, and may require bilingual abilities (Spanish, Chinese, or Bengali) in some areas. Salary ranges from $110,000 to $115,000 with benefits and a $10,000 sign-on.

Qualificações

• NYS RN license required.
• CCM is preferred.
• 2+ years RN Case Management in managed care MLTC.
• Bilingual ability required in some areas (Spanish, Chinese or Bengali).
• Experience in Home Care, MLTC, or hospital case management is beneficial.

Responsabilidades

• Provide telephonic and in-person case management to members.
• Develop, implement and monitor care plans in coordination with PCPs and caregivers.
• Perform field visits approximately 1 day per week in designated areas.
• Collaborate with an interdisciplinary team to meet patient needs.
