Matter of Care is seeking a Clinical Assessment Manager for care delivery in Brooklyn. The role involves leading comprehensive in-home assessments, evaluating eligibility for long-term care services, and coordinating with care teams to connect older adults with needed supports.

The position requires an active NYS RN license, 5+ years of nursing experience, and the ability to travel across Brooklyn. It offers a daytime schedule and a hybrid work option after training.

Qualifications

• Active New York State RN license.
• Minimum 5 years of clinical nursing experience.
• Experience in managed care, PACE, MLTC, home care, case management, utilization management, population health, or related settings.
• Ability to travel throughout Brooklyn.

Responsibilities

• Conduct comprehensive in-home clinical assessments using the UAS-NY.
• Evaluate participant eligibility and ongoing care needs for community-based long-term care services.
• Perform reassessments and identify changes in condition, function, diagnoses, and medications.
• Develop proposed service plans and recommendations to support health and independence.
• Provide hands-on nursing services in participants’ homes as needed within scope of practice.
• Collaborate with Enrollment, Intake, Care Delivery, and IDT teams.
• Document clinical findings accurately in EMR systems.
• Participate in quality improvement initiatives and serve as a clinical resource.
