Job summary

Groups Recover Together is seeking a Recovery Support Coordinator in Martinsville, VA. This hybrid, full-time role supports 160–180 members with health-related social needs, treatment engagement, and seamless care coordination. You will work with a multidisciplinary team and report to the Recovery Operations Supervisor.

Your responsibilities include member coaching, care navigation, outreach, and documentation in the EHR, with evening hours as needed and a pay range of $43,000–$46,000.

Qualifications

• High school diploma required; relevant associate degree preferred.
• Bachelor’s degree in related field may be required in some states (Virginia and Tennessee).
• Associate’s degree with one year experience or MHRT/C or Community Health Worker certification may be required in select programs or states.
• Minimum of two years’ experience in case management, care coordination, or peer support preferred; work with substance use or mental health challenges is a plus.

Responsibilities

• Provide recovery support and care coordination with a multidisciplinary team.
• Build trusted relationships and coach members toward recovery goals.
• Assist with navigating healthcare, housing, insurance, and transportation needs.
• Maintain accurate EHR documentation and support group facilitation logistics.
• Engage in community outreach and education as needed.
