Job summary

Groups is an outpatient provider delivering SUD treatment with a blend of medication, therapy, and personalized support. The Recovery Support Coordinator helps members engage in recovery by coordinating care across clinicians, social services, and community resources in Dunbar, WV.

Responsibilities include one-on-one coaching, outreach to re-engage clients, and managing referrals while working evenings as needed.

Qualifications

• Minimum of two years’ experience in case management or care coordination.
• Experience with SUD or mental health challenges is preferred.
• Experience using Electronic Health Records (EHR) is required.

Responsibilities

• Coordinate care across clinical, social, and community settings to support member recovery goals.
• Build trust and assist members in achieving their recovery goals through empathy and motivational interviewing.
• Provide in-person, virtual, and telephonic support based on member needs and regional availability.
• Maintain a shared caseload with the care team to ensure coordinated service delivery.
