Job summary

Groups is a leading outpatient provider focused on supporting individuals with substance use disorders. The Recovery Support Coordinator will manage a panel of 160–180 members, coordinating care across clinical and community settings, and provide recovery support and navigation services.

Responsibilities include member engagement, care coordination, and linking members to healthcare and social services, with hybrid work and a salary in the mid-40k range.

Qualifications

• High school diploma required; Associate’s degree in Social Services, Human Services, Psychology, or related field preferred.
• State-specific additional education requirements vary by state; may include Bachelor’s degree in related field (VA and TN).
• Peer Support Specialist / Peer Recovery Coach certification may be preferred or required in select programs or states.

Responsibilities

• Collaborate with a multidisciplinary care team to ensure high-quality, member-centered care.
• Build trusted relationships with members to drive recovery goals.
• Provide coaching, coping skill support, and engagement in recovery activities.
• Support re-engagement of unengaged members through outreach.
• Assist members in navigating insurance, housing, transportation, and other resources.
• Manage referrals and documentation to facilitate transitions of care.
• Promote recovery-positive language and connect members to community resources.
