Job summary

UnitedHealth Group is seeking an Associate Patient Care Coordinator in Worcester, MA. The role supports the behavioural health department and partners with care teams to deliver excellent patient service in a hybrid work model after training.

Responsibilities include accurate scheduling, patient education, documentation in Epic EMR, and collaboration with staff and providers to meet service goals. Training onsite for 12 weeks is required.

Qualifications

• High School Diploma or GED required.
• 1+ year of experience in a healthcare setting.
• 1+ year of call center experience.
• Ability to complete 12 weeks of onsite training in Worcester, MA.

Responsibilities

• Verifies and updates patient demographics and information.
• Schedules patients according to department protocols.
• Completes patient check-out and coordinates scheduling.
• Provides education and pre-appointment instructions.
• Documents information in the electronic medical record system.
• Handles insurance referrals per protocol.
• Escalates urgent patient issues to nursing or provider as needed.
