Qualifications

• High school diploma or GED.
• 2+ years in patient- or customer-facing roles with measurable outcomes.
• Ability to pass hospital credentialing, including vaccinations.
• Independent work on-site with limited supervision.
• Proficiency with CRM/case management systems and iPad/iPhone.

Responsibilities

• Identify and approach eligible patients and families from hospital referrals.
• Explain HIPP clearly and compassionately, adapting communication to each family.
• Work through hesitation and mistrust without pressuring anyone.
• Gather authorizations and documentation to move an application forward.
• Own caseload from first contact to enrollment.
• Explain employer coverage and Medicaid together and verify ongoing eligibility.
• Manage referrals and patient records in PFA's CRM; document interactions compliant with HIPAA.
• Represent PFA on-site as the primary hospital contact and build relationships with staff.
