Job summary

Patient Funding Alternatives (PFA) seeks a Patient Advocate Specialist for a full-time hybrid role at New York Presbyterian/Columbia University Irving Medical Center. You will educate eligible hospital patients about the Health Insurance Premium Payment program and help enroll them into employer-sponsored coverage alongside Medicaid.

You’ll manage referrals, collect necessary documentation, and coordinate with hospital staff while maintaining HIPAA standards.

Qualifications

• High school diploma or GED required.
• Full bilingual proficiency in Spanish required.
• 2+ years in a patient- or customer-facing role with activity/outcome measurement.
• Independent work on-site with limited supervision.
• CRM or case management system experience; comfortable with iPad/iPhone.
• Ability to pass hospital credentialing and screenings.

Responsibilities

• Engage and advocate for patients and families from hospital referrals.
• Explain HIPP and enrollment options clearly and compassionately.
• Address hesitation and objections while avoiding pressure on families.
• Gather documentation to move applications forward and maintain professional boundaries.
• Own caseload from first contact to enrollment and hand off to relevant teams.
• Manage referrals and patient records in the CRM, upload and transmit documentation securely.
• Represent PFA on-site as the primary hospital contact and build relationships with staff.
• Operate in a metrics-driven environment with weekly/monthly targets and reporting.
