Qualifications

• High school diploma or GED required.
• Full bilingual proficiency in Spanish.
• 2+ years in a patient- or customer-facing role with measurable activity or outcomes.
• Ability to work on-site independently with limited daily supervision.
• Experience with a CRM or case management system; comfortable using iPad and iPhone.
• Ability to pass hospital credentialing and screenings.

Responsibilities

• Identify and approach eligible patients and families from hospital referrals.
• Explain HIPP clearly and compassionately; adapt communication to each family.
• Gather authorizations and documentation to move an application forward.
• Own caseload from first contact to enrollment.
• Maintain referrals and patient records in CRM; ensure HIPAA compliance.
