Responsibilities

• Evaluates the needs of the member, resources, and recommends the plan for the best outcome.
• Develops ongoing long-term care plans and identifies providers and community resources.
• Coordinates between member/family and care provider team to ensure services are accessible.
• Provides resource support for housing, employment, independent living, and related needs.
• Monitors care plans and outcomes and makes recommendations.
• Interacts with providers to ensure member needs are met.
• Ensures compliance with state, federal, and third-party payer regulations.
