Community of Hope is seeking a Housing Stability Case Manager (Temporary) in Washington, DC, offering hybrid work arrangements. This role supports families to secure and maintain housing, increase self-sufficiency, and strengthen family stability, with an anticipated end date of April 30, 2027.

The position requires experience in housing/homelessness services, strong resource knowledge, and excellent communication.

Qualifications

• Associate's degree or two years of college required.
• Minimum two years of experience with homeless services programs or social services.
• Minimum of two years of experience in housing/homelessness or social services field.
• Strong knowledge of community resources or ability to become knowledgeable.
• Strong interpersonal and organization skills.
• Strong communication skills including verbal and writing.
• Proven ability to submit timely, detailed and accurate case notes.

Responsabilités

• Develops individual goal plans with families using a two-generation approach and updates plans as needed.
• Meets with caseload families to support plan implementation and follow-up.
• Networks with community resources, makes referrals, and tracks participation in housing, employment, medical care, and services.
• Assists families with housing search, accompanies to view units, advocates with landlords, and helps with applications.
• Conducts scheduled and unscheduled home visits and provides tenant education.
• Maintains regular communication with families and documents progressive engagement efforts.
• Operates within a housing-first, strengths-based, client-centered framework.
