Job summary

Community of Hope in Washington, DC is seeking four full-time Housing Stability Case Managers for our Rapid Rehousing program. You will help families secure and maintain housing, increase self-sufficiency through income, benefits, and education, and strengthen family stability in a mission-driven setting.

Position is hybrid-based at MLK Jr Ave SE and involves coordinating with Housing Specialists, performing home visits, and providing tenant education within contract guidelines.

Qualifications

• Bachelor’s degree in social work or related field required or 2 years’ experience in a social services and/or human service field.
• Minimum one year work experience in social service or related field required with degree. Minimum two years work experience in social service field if no degree.
• Strong interpersonal, conflict-resolution, and organizational skills.
• Ability to work independently and as a team.
• A valid driver’s license and regular access to a car, as well as proof of auto insurance, required for staff working in scattered site housing programs.
• Nice-to-haves: Prior experience working with individuals or families experiencing homelessness and/or living in poverty strongly.

Responsibilities

• Develops individual goal plans for families using a two-generation approach with measurable action steps.
• Meets with families on caseload to support goal plan implementation and follow-up.
• Networks and coordinates with community resources; makes referrals and tracks participation in housing, employment, medical, and other services.
• Assists families with housing search and, if needed, accompanies them to view units and completes applications.
• Conducts scheduled and unscheduled home visits and provides tenant education.
• Maintains regular communication with all families on caseload and documents progressive engagement efforts.
• Works within a housing-first framework to connect clients with immediate housing and supportive services.
