Job summary

Community of Hope in Washington, DC is seeking a Family Support Specialist to provide home visitation, family education, and resource coordination for at-risk families. You will manage a caseload under PAT guidelines, develop family plans, and document progress in the DC system.

You will work hybrid in DC, meet weekly with supervisors, and connect families with housing, health, and social services while maintaining strict program documentation and privacy standards.

Qualifications

• High school diploma is required.
• Valid CPR & First Aid Certification.
• Minimum of 2 years of experience in public and/or human services.
• Bilingual in English and Spanish or Amharic.
• Ability to conduct home visits with a valid driver's license and auto insurance.
• Knowledge of community resources and the ability to learn new ones.
• Work flexible hours including weekends and evenings.
• Vaccination proof required; accommodations considered per law.

Responsibilities

• Provides home visitation services meeting DC Health grant and PAT curricula.
• Maintain caseload of 15-25 families enrolled in PAT Home Visiting program.
• Develop Family Support Plans within one month of enrollment; update monthly with manager.
• Complete assessments and screenings (ASQ-3, ASQ: SE-2, PHQ-9, IY, AB Inventory).
• Documentation: notes, releases, incidents, referrals; scanned in DCRS.
• Network with COH and community resources; document progress in records.
• Weekly 90-minute reflective supervision with PAT Program Coordinator.
• Refer to Strong Start or Early Stages following screenings as needed.
