Job summary

Northern Virginia Family Services (NVFS) is seeking a Case Manager to support individuals and families facing homelessness through the RHAP program. You will conduct screenings, develop service plans, and connect clients with housing, healthcare, employment and community resources.

Role emphasizes client-centered planning, documentation, and collaboration with landlords and partners. A background in human services and at least one year of case management is preferred; Hybrid work and client-site

Qualifications

• Bachelor’s degree in Social Work or related field, OR
• Associate’s degree with two years of related work experience OR
• at least four years of related work experience with no relevant education.
• At least one year of case management experience in human services; housing crisis experience preferred.
• HMIS experience preferred; ability to maintain accurate reports and documentation.
• English fluency required; Spanish fluency preferred.

Responsibilities

• Screen clients and determine eligibility for housing assistance and services.
• Provide individualized case management and develop service plans.
• Connect clients with housing, healthcare, employment, education and other resources.
• Conduct home visits and reassess progress to support stability.
• Collaborate with landlords and community organizations to place clients in housing.
