Job summary

Northern Virginia Family Services (NVFS) is seeking a Case Manager to support the Rapid Rehousing program within the Community Housing team. You will conduct screenings, provide individualized case management, and build partnerships with landlords and community organizations to secure and maintain safe housing for adults and families experiencing homelessness or housing instability.

Qualifications

• Bachelor’s degree in Social Work or related human services field or equivalent experience.
• Experience in housing-related crisis and case management preferred.
• HMIS experience preferred.
• Strong documentation and reporting skills.
• Ability to communicate with diverse client base and landlords.

Responsibilities

• Conduct client screenings, assessments, and intake to determine eligibility for housing assistance and supportive services.
• Provide individualized case management, develop service plans, and assist clients in achieving housing, financial, employment, and personal goals.
• Coordinate housing opportunities with landlords, property managers, and community organizations.
• Maintain accurate client records, notes, and required program documentation; ensure compliance with guidelines.
