Total Care Services, Inc. seeks a Supports Planner in Maryland to identify, access, and coordinate services that maintain clients’ health, safety, and independence. The role also handles waiver eligibility and plan maintenance.

Requirements include a Master’s or Bachelor’s in Human Services, 21+ years old, valid driver with insurance, US work authorization, and solid Word/Excel skills. This full-time, hybrid position includes robust benefits and PSLF eligibility.

Qualifications

• Minimum of 21 years of age.
• MUST have Masters or Bachelors degree in Human services field.
• Must be a licensed driver and have own transportation with valid insurance.
• Must be a U.S. citizen or alien who is lawfully authorized to work in the United States.
• Must have computer skills and know how to perform tasks utilizing Microsoft Word, and MS Excel.
• Must have excellent oral and written communication skills.
