Job summary

Ethos, a Boston-based nonprofit, seeks a Geriatric Support Service Coordinator to manage home- and community-based services for older adults. The role is full-time and hybrid, with some work-from-home flexibility, based in Boston, MA.

You will conduct assessments, craft person-centered care plans, coordinate with families and providers, and promote safety, dignity, and independence. Responsibilities include documenting care, participating in interdisciplinary discussions, resolving service

Qualifications

• Experience in geriatric care coordination or human services.
• Strong assessment and care planning skills.
• Effective communication with consumers, families, caregivers, service providers and interdisciplinary teams.
• Knowledge of community-based services for aging and disability support.
• Ability to work respectfully with individuals from diverse cultural, linguistic, socioeconomic, and LGBT backgrounds.
• Proficiency with electronic records and basic office software.

Responsibilities

• Coordinate home and community-based services for older adults.
• Conduct assessments to determine needs and risks.
• Develop person-centered care plans promoting safety and independence.
• Communicate with consumers, caregivers, and service providers; monitor service delivery.
• Document activities and participate in interdisciplinary case discussions.
• Educate consumers and families about available resources and advocate for culturally competent care.
