Job summary

Mass General Brigham Community Physicians is seeking a GRACE Social Worker to perform in-home geriatric assessments and develop care plans with the GRACE team. The role involves coordinating with primary care providers, offering psychosocial support, and engaging patients and families.

The position is full-time, hybrid, in the Boston area, with remote work part of the week and travel to patient homes as needed.

Qualifications

• MSW from an accredited school of Social Work is required.
• Driver's license required.
• 2+ years of clinical experience in a medical setting.
• 1+ years with geriatric patients preferred.
• Clinical licensure preferred.

Responsibilities

• Conduct in‑home geriatric assessments with GRACE NP.
• Develop and implement care plans using GRACE protocols.
• Engage patients and caregivers in psychosocial support and education.
• Coordinate care with primary care providers and GRACE team.
• Provide ongoing home-based care management and coordination.
• Participate in weekly interdisciplinary team meetings.
