WellBe Senior Medical is seeking a Patient Care Coordinator to join our home-based care team. The role focuses on coordinating care for frail, chronically ill patients, scheduling appointments, and maintaining EMR documentation with a patient-centric approach.

You will work across in-home, hospital, and facility settings, collaborating with clinicians to ensure complete care and timely communication. This hybrid position offers a dynamic environment and opportunities to improve patient outcomes.

Qualifications

• High school graduate or GED.
• MA Certification-Preferred.
• 4+ years in healthcare setting is implied by responsibilities.

Responsabilités

• Answer incoming and outgoing calls; coordinate care with medical team.
• Schedule patient appointments and diagnostic tests; coordinate referrals.
• Maintain and update digital records; ensure EMR accuracy.
• Provide patient-focused service; act as community resource.
• Engage unengaged patients and support quality of care initiatives.
• Prepare charts and assist with team meetings and reporting.
