Job summary

Selfhelp Community Services seeks a dedicated professional to provide assessment, service coordination and counseling for geriatric clients eligible for community services. The role includes intake, ongoing advocacy, in-home assessments, crisis intervention, and referrals.

You will prepare reports, maintain accurate files, and collaborate with supervisors and nurses to coordinate care. This position requires strong communication, customer service, and attention to detail.

Qualifications

• Bachelor's degree and related experience required.
• Strong communication and listening abilities are essential.
• Experience with case management and service coordination preferred.

Responsibilities

• Assess strength and weaknesses of resources and provide case management.
• Coordinate intake, advocacy, in-home assessments, crisis intervention, and referrals.
• Prepare reports and maintain client files with documentation.
• Maintain relationships with partner organizations to secure services.
• Collaborate with supervisors and nurses to coordinate client care.
• Participate in trainings and supervision; report needs and issues.
• Attend meetings and regulatory training to ensure compliance.
• Perform other duties as needed.
