Job summary

Old Colony YMCA is seeking a dedicated Geriatric Support Service Coordinator (GSSC) to help determine eligibility and ensure provision of services for SCO members. You will assess health and functional status, develop individualized care plans, and promote independent living within the least restrictive environment.

The role requires at least two years in human services, a BA/BS in social work or related field, and strong communication.

Qualifications

• Two years of experience in human services, preferably with the elder population.
• Bachelor’s degree in social work or related field; licensure if appropriate.
• Adaptability to changing consumer and program needs.
• Strong written and verbal communication skills.
• Bilingual preferred (Cantonese; Haitian Creole).

Responsibilities

• Promote independence and provide services in least restrictive setting.
• Conduct initial and ongoing assessments and develop care plans.
• Build rapport and coordinate care with consumers.
• Provide regular contact, including home visits, to monitor progress.
• Maintain accurate documentation.

Education

Bachelor's degree in social work

Two years of experience in human services
