Job summary

Agency on Aging of South Central Connecticut is seeking a Care Manager to support older adults living independently by creating individualized care plans and coordinating services across healthcare and community partners. The role emphasizes person-centered support and collaboration with families and providers.

You'll advocate for clients, manage discharge planning, and document thoroughly to meet regulatory standards, with a flexible hybrid schedule and weekday hours.

Qualifications

• Bachelor’s degree in Human Services, Social Work, or related field.
• At least 2 years of experience in case management or similar role.
• Strong understanding of Medicaid regulations and care coordination.
• Excellent communication, organizational, and interpersonal skills.
• Compassionate, patient, client-centered approach.

Responsibilities

• Develop and manage personalized care plans based on client assessments.
• Coordinate hospital discharge planning, mental health support, and long-term care options.
• Collaborate with family members, healthcare providers, and interdisciplinary teams.
• Advocate for clients and connect them to community resources.
• Maintain documentation in compliance with agency and regulatory standards.
