Job summary

Qualifications

• 2+ years of clinical experience in a medical setting.
• Experience with geriatric patients preferred.
• Ability to conduct in-home assessments and coordinate care.

Responsibilities

• Conduct in-home geriatric assessments with GRACE Nurse Practitioner.
• Develop and implement individualized care plans using GRACE protocols.
• Coordinate with primary care providers and GRACE team to optimize outcomes.
