Enlyte is seeking a Field Case Manager in the Winston-Salem, NC area for a full-time, hybrid role. You will partner with physicians, employers, and patients to create effective treatment plans that restore work ability while delivering cost-efficient healthcare services.

The role requires an RN license in good standing and 2+ years of clinical practice, with travel to in-person appointments in assigned geography. Comprehensive benefits and training are provided.

Qualifications

• RN license in good standing.
• 2+ years of clinical practice preferred.
• URAC-recognized case management certification (CCM, CDMS, CRC, etc.) preferred.

Responsibilities

• Apply case management standards of practice to plans.
• Advise on treatment plans with physicians and employers.
• Interview patients to assess recovery and work readiness.
• Collaborate with physicians and workplaces to tailor plans.
• Attend appointments and prepare status updates for customers.
