Job summary

Enlyte is seeking a Field Case Manager in the Franklin, TN area for a full-time hybrid role requiring travel to in-person appointments. You will coordinate care with treating physicians, employers, and patients, and develop individualized treatment plans aimed at returning patients to work.

The role requires RN licensure, 2+ years of clinical experience, and URAC‑recognized case management certification. A comprehensive benefits package and travel support are provided.

Qualifications

Education: Associates or Bachelor’s in Nursing or related field.
Experience: 2+ years clinical practice preferred.
Licenses/Certifications: Active RN license; URAC‑recognized CCM/CDMS/CRC/CRRN or related.
Travel: Must travel to in-person appointments with injured workers.

Responsibilities

Develop and implement treatment plans for injured/disabled patients.
Collaborate with physicians, employers, and facilities to enable early return to work.
Assess recovery progress and adjust goals and care plans accordingly.
Manage workload, prepare status updates, and attend appointments as needed.
