UnitedHealth Group is seeking a Field Care Coordinator to serve a panel of members with varied medical and behavioral needs. You will perform comprehensive assessments, develop person-centered care plans, and collaborate with care teams to improve health outcomes.

The role is full-time (40 hours/week), field-based with a home-based office. You may work from home when not in the field, and occasional overtime may be required during 8am–5pm hours.

Competenze

• Bachelor's degree or higher in a health-related field or equivalent.
• 2+ years of healthcare experience in a relevant setting.
• LCSW/LMSW/LMHC licensure or certification preferred.
• Proficient in MS Office (Word, Excel, Outlook).
• Valid driver’s license and access to reliable transportation.

Mansioni

• Engage members face-to-face or by telecommunication to complete assessments.
• Develop and implement person-centered care plans for medical and behavioral needs.
• Collaborate with internal care teams, providers and community resources.
• Educate and coach members to support self-management of health and lifestyle changes.
• Coordinate care transitions following ER visits or SNF stays.
• Advocate for member needs with the health care team and family.
• Establish a positive, respectful relationship with members.
• Learn and apply cultural sensitivity and health literacy best practices.
• Conduct Comprehensive Needs Assessments (CNA).

Conoscenze

MS Office (Word, Excel, Outlook)

Driver's license

Travel flexible (up to 50%)

Formazione

Bachelor's degree or higher (healthcare related)

2+ years of relevant healthcare experience

Licensure/Certification: LCSW/LMSW/LMHC preferred
