Job summary

Able Health Services Inc. is seeking a Coordinator of Community Services to join our team. The role focuses on providing Targeted Case Management to the individuals we support and coordinating resources with their teams to develop and monitor a Person-Centered Plan (PCP).

The position emphasizes advocacy, conflict resolution, and timely documentation of activity notes. It requires strong collaboration with the person supported and a commitment to self-determination principles, with hybrid work

Qualifications

• Bachelor’s degree in a human services field.
• Associate’s degree with 2 years of human services experience.
• 7 years of experience in the human service field.

Responsibilities

• Coordinate targeted case management and related supports.
• Advocate for individuals and coordinate resources and services.
• Develop and monitor a Person-Centered Plan (PCP) with the team.
• Visit assigned individuals for PCP activities at least three times per year.
• Assist with transitions and transfers to maintain continuity of supports.

Education

Bachelor’s degree in human services

Associate’s degree + 2 years experience in human services

7 years experience in human services
