Job summary

Able Health Services Inc is seeking a Part-time Coordinator of Community Services to join our team. This role provides targeted case management, advocacy, and coordination of supports for the individuals we serve, with emphasis on person-centered planning and timely documentation.

The position requires a bachelor’s or equivalent experience in human services, a valid driver’s license, and strong communication and collaboration skills.

Qualifications

• Bachelor’s degree or equivalent experience in human services.
• Strong advocacy and relationship-building abilities.
• Proficient written and oral communication; skilled with documentation.
• Access to reliable automobile and valid driver’s license.

Responsibilities

• Coordinate activities to plan, access, and maintain supports desired by the individual.
• Advocate for individuals and assist them to advocate for themselves.
• Promote participation in the team process and empower individuals to lead meetings.
• Negotiate and resolve conflicts within the team.
• Ensure Person-Centered Plans align with COMAR and agency policy; monitor implementation.
• Visit individuals at least three times per PCP year; support transitions to continue services.
• Document activities and ensure proper billing; maintain required protocols.
