Job summary

Person Centered Services Care Coordination Organization is seeking a Care Coordinator to join our health home model, advocating for individuals with intellectual and developmental disabilities and coordinating across health, behavioral, and community supports.

The role emphasizes a person-centered, holistic approach with collaboration among care teams; hybrid work is available and a flexible schedule supports both full-time and part-time options.

Qualifications

• Bachelor's degree with 2 years relevant experience OR a Licensed Registered Nurse with 2 years relevant experience OR Master's degree with 1 year of relevant experience.
• Minimum of a Bachelor's degree is required by OPWDD in NYS

Responsibilities

• Develops a comprehensive, person-centered Life Plan with the individual and their circle of support, as well as their entire service provider team.
• Effectively manage a tiered caseload, while tailoring services to individual needs. Similar responsibilities as a Care Manager.
• Completes enrollment and eligibility documentation.
• Meets with individuals in their homes, physician/provider offices, and other public places to conduct assessments and provide services.
• Commits to a respectful, just, and supportive environment for individuals and coworkers aligning with the company’s commitment to diversity, equity, and inclusion.
