Job summary

Person Centered Services Care Coordination Organization, LLC seeks a dedicated Care Coordinator to coordinate health, behavioral health, and community supports for individuals with IDD. You will advocate for needs across providers using a person-centered approach to ensure comprehensive care.

Responsibilities include developing Life Plans, managing caseloads, enrolling clients, and conducting home visits. Hybrid work enables flexibility while maintaining in-person onboarding.

Qualifications

• Bachelor’s degree with 2 years of relevant experience or RN with 2 years’ experience or Master’s with 1 year of relevant experience.
• Relevant experience includes direct care, coordination, or advocacy for individuals.
• License requirements per NYS OPWDD for the role.

Responsibilities

• Develop a comprehensive Life Plan with the individual and support network.
• Maintain a respectful, inclusive environment aligned with DEI values.
• Manage a tiered caseload and tailor services to needs.
• Complete enrollment and eligibility documentation.
• Meet individuals in homes or offices to conduct assessments and provide services.
• Work from home as needed and report to office at least weekly.
