Qualifications

• Minimum of 21 years of age.
• Must have Masters or Bachelors degree in Human services field.
• Must be a licensed driver with own transportation and valid insurance.
• Must be a U.S. citizen or authorized to work in the United States.
• Must have computer skills and know how to use MS Word and MS Excel.
• Excellent oral and written communication skills.

Responsibilities

• Identify, access, and coordinate services and supports to maintain client health, safety, and independence.
• Assist with waiver eligibility determination and maintenance.
• Advocate for clients and help them understand options and develop a plan of service.
• Coordinate with providers to arrange delivery of services and supports.
• Monitor and resolve issues related to client services.
