Job summary

Total Care Services, Inc. in Maryland is seeking dedicated Case Managers to support individuals with intellectual and developmental disabilities in our Coordination of Community Services program.

You will work from your home office with local community visits in a hybrid schedule. The role requires a Bachelor's in human services, 1–3 years of clinical case management, a valid driver’s license, and up-to-date vaccinations.

Qualifications

• Bachelor's degree in human services field.
• 1 to 3 years of Clinical Case Management.
• Must be a licensed driver with reliable transportation, valid insurance, and a safe driving record.
• Fully vaccinated and pass all pre-employment qualifications.

Responsibilities

• Collaborate with clients and families to identify needs, goals, and preferences.
• Provide information about community resources and support services.
• Empower clients to make informed choices and plan for their future.
• Assist clients in accessing service delivery systems, resources, and supports.
• Monitor and evaluate services to ensure they meet clients' needs and preferences.
• Work environment is hybrid with home office and local community visits.
