This hybrid role combines remote planning from home with local community visits to deliver high-quality care and coordination of services. You will manage client plans, connect families to resources, and ensure services align with each client's goals while meeting billable hour targets in LTSSMaryland.

Qualifications

• Bachelor's degree in human services field.
• 1–3 years of Clinical Case Management experience.
• Must be a licensed driver with reliable transportation and a valid license.
• Fully vaccinated and pass pre-employment checks.

Responsibilities

• Identify needs, goals, and preferences with clients and families.
• Provide information about community resources and support services.
• Empower clients to plan for their future and make informed choices.
• Assist clients in accessing service delivery systems and supports.
• Monitor and evaluate services to ensure they meet client needs.
