Responsibilities

• Provide information and education about the Medicaid waiver process
• Create an Individual support plan based on the individual’s strengths, needs, goals, and desires
• Help individuals access services and supports to meet their goals
• Facilitate regular community-based team meetings to review plans of care with providers
