Job summary

Indiana Professional Management Group in Indiana is seeking a full-time Case Manager for a hybrid role combining home-based work with travel to community meetings within Hamilton County and surrounding areas. Candidates must reside in Hamilton County and be able to travel for meetings.

Responsibilities include educating on Medicaid waivers, developing individualized plans, helping clients access services, and coordinating with other providers.

Qualifications

• Bachelor’s degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field.
• A valid driver’s license, car insurance, and reliable transportation.
• Ability to pass thorough background screens.
• Designated home office with computer, internet, and smartphone.

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process.
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve.
• Help Individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed.
