Qualifications

• Bachelor's degree required or equivalent experience in human services fields.
• Valid driver's license, car insurance, and reliable transportation.
• Ability to pass background screens.
• Designated home office with computer and internet access.

Responsibilities

• Provide information and education to help individuals understand the Medicaid waiver process.
• Create an Individual support plan based on strengths, needs, goals, and desires of the individuals served.
• Help individuals access services and supports to meet their goals.
• Facilitate regular community-based team meetings to review plan of care with providers.
