IPMG in Indiana is seeking a full-time Case Manager to serve Henry County, with a hybrid schedule combining remote work from home and in-person community meetings several times a week. Applicants must reside within Henry County and be able to travel throughout the county and surrounding areas.

You will provide information about Medicaid waiver processes, create individualized support plans, help Individuals access needed services, and facilitate regular community-based team meetings to

Formación

• Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field.
• Bachelor's degree in an unrelated field and a minimum of one year of direct experience with people with IDD or two years with aging population.
• Valid driver's license, car insurance, and reliable transportation.
• Ability to pass background screens.
• Dedicated home office with computer, internet, and smartphone.

Responsabilidades

• Provide information and education to help Individuals understand the Medicaid waiver process.
• Create an Individual support plan based on strengths, needs, goals, and desires of the Individuals we serve.
• Help Individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed.
