Job summary

Indiana Professional Management Group, Inc. is hiring for a Case Manager in a full-time hybrid role serving St Joseph County.

You will educate individuals about Medicaid waivers, develop individual support plans, assist in accessing services, and lead community-based team meetings with providers. Applicants must have a relevant bachelor's degree or equivalent experience, a valid driver's license, and be able to travel within the county and surrounding areas for meetings.

Qualifications

• Bachelor's degree in psychology, sociology, social work, or related human services field.
• Alternate bachelor’s degree plus 1 year direct experience with individuals with IDD, or 2 years with aging population.
• Valid driver's license, car insurance, and reliable transportation.
• Ability to pass background checks.
• Designated home office with computer, internet, and smartphone.

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process.
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve.
• Help Individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed.
