Job summary

Indiana Professional Management Group in Indiana is seeking a Case Manager to join a full-time hybrid role serving Hamilton County. You will educate individuals about Medicaid waivers and develop plans of care in collaboration with a multidisciplinary team.

This position requires a bachelor’s in a human services field or related experience, reliable transportation, ability to travel for community meetings, and the capacity to pass background checks.

Qualifications

• Bachelor’s degree in a human services field or related field.
• Ability to obtain reliable transportation and valid driver's license.
• Willingness to travel within Hamilton County and surrounding counties.
• Pass background screening.

Responsibilities

• Explain Medicaid waiver process to individuals.
• Create an individual support plan based on the individual’s strengths, needs, goals and desires.
• Help individuals access services and supports to meet goals.
• Lead community-based meetings to review plans and coordinate with providers.
