Job summary

Indiana Professional Management Group, Inc. in Indiana is seeking a Case Manager for a full-time hybrid role, serving St Joseph County. The position combines remote work from your home office with travel to community-based meetings several times a week.

You will educate individuals about Medicaid waivers, develop individualized support plans, assist in accessing services, and coordinate regular team meetings with providers to review care plans.

Qualifications

• Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field.
• A Bachelor's degree in an unrelated field and a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population.
• A valid driver's license, car insurance, and reliable transportation.
• The ability to pass thorough background screens.
• Designated home office setting with computer, internet service, and smart phone

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve
• Help Individuals access services and supports they need to meet their goals
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed

Education
