Job summary

IPMG in Indiana is seeking a full-time hybrid Case Manager serving St Joseph County. You will work from a home office with local travel for community meetings several times a week and must reside within the county.

This role supports individuals across Medicaid waivers and requires dependable transportation. You’ll educate participants about the waiver process, develop Individual Support Plans, coordinate services, and lead community-based team meetings with providers.

Qualifications

• Valid driver's license, car insurance, and reliable transportation.
• Able to pass thorough background screens.
• Home office setup with computer, internet and smartphone.

Responsibilities

• Provide information and education about the Medicaid waiver process.
• Create an Individual Support Plan based on client strengths and goals.
• Help Individuals access services and supports to meet goals.
• Facilitate regular community-based team meetings to review plans with providers.
