Job summary

IPMG, serving Vanderburgh County, Indiana, is seeking a Case Manager for a full-time hybrid role combining home-based work with in-person meetings within the county. You’ll travel to community settings several times a week and coordinate with service providers to support individuals under four Medicaid waivers.

Requirements include a related bachelor’s degree or equivalent experience, a valid driver’s license, and the ability to pass background screens.

Qualifications

• Bachelor's degree in a human services field or related experience.
• Valid driver’s license, car insurance, and reliable transportation.
• Ability to pass thorough background screens.
• Designated home office with computer, internet and smart phone.

Responsibilities

• Provide information and education to help individuals understand the Medicaid waiver process.
• Create an Individual support plan based on strengths, needs, goals and desires of the individuals served.
• Help individuals access services and supports to meet their goals.
• Facilitate regular community-based team meetings to review plan of care with providers.

Education

Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field

Bachelor's degree in an unrelated field + 1 year direct experience with people with intellectual/developmental disabilities OR 2 years with aging population
