Indiana Professional Management Group is seeking a Case Manager for a full-time hybrid role serving Allen County, Indiana. The position combines remote work from your home office with travel for in-person community meetings several times per week.

You’ll educate individuals about Medicaid waivers, develop individualized plans, help access services, and lead team meetings with care providers. A relevant bachelor’s degree is required, with specific fields listed; travel and home office setup are

Qualifications

• Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation or another human services field.
• Bachelor's degree in an unrelated field with at least 1 year of direct experience with individuals with disabilities or 2 years with aging population.

Responsibilities

• Provide information and education to help individuals understand the Medicaid waiver process.
• Create an Individual support plan based on strengths, needs, goals, and desires of the individuals we serve.
• Help individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed
