Job summary

Indiana Professional Management Group, Inc. seeks a Case Manager to join our Hamilton County team in a full-time, hybrid role.

You’ll provide information on Medicaid waivers, build individualized support plans, help clients access services, and participate in community-based team meetings. Applicants must reside in Hamilton County, Indiana, have a relevant bachelor's degree or equivalent experience, and be willing to travel within the county and surrounding areas for in-person meetings.

Qualifications

• A Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field, or
• A Bachelor's degree in an unrelated field and a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population
• A valid driver's license, car insurance, and reliable transportation
• The ability to pass thorough background screens
• Designated home office setting with computer, internet service, and smart phone

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve
• Help Individuals access services and supports they need to meet their goals
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed

Education
