Job summary

Boston Children's Hospital is seeking a Patient Navigator to support enrollment and data collection in the EHR, collaborating with the HRSN enrollment team and hospital-wide partners. Qualifications

• High School Diploma / GED is required.
• Bachelor's Degree in Human Services or related field preferred.
• Experience with the clinic population or community resources preferred.
• Fluency in Haitian Creole, Cape Verdean Creole and/or Spanish strongly preferred.

Responsibilities

• Meets with/calls patients and families to review care plans and refers to clinic and community services.
• Assesses patients for referrals to resources and acts as liaison with internal and community resources.
• May assist scheduling multidisciplinary appointments and coordinating transportation.
• Assists patients with applying for insurance and determines eligibility for services.
• Maintains relationships with providers and community resources; reports outcomes of referrals.
• Documents patient care and collects required data; supports planning and develops data.
• Attends care planning meetings to prepare patients for discharge or continuing care.
